BSA Physical Forms:All Scouting participants MUST have physical forms filled out to participate in Troop activities. You can download blank forms below. Make sure you take a blank form with you to your physical. Please plan ahead, some doctor offices take quite a while to get into! The expiration date is with respect to the date the Doctor signed the physical. Class 1 & 2 Physical FormClass 1 Physical: Is on the rear of the Scout application form (and the first page on the Class 1 & 2 form). This form is good for all aged Scouts and adults for weekend camping and other short term Scouting activities and can be filled out by the adult leader/parent/guardian. Class 2 Physical: Required once every 36 months for all participants less than 40 years of age. This form MUST be filled out and signed by a doctor for summer camp, NYLT and other long term Scouting activities. If your son has had a physical within the last 36 months, most doctors’ offices will fill out this form for you if you provide them a blank copy. If you already filed one of these with the Troop for 2006 summer camp, the parent/guardian can sign and date the same form for 2007 – indicating that nothing medically has changed for your son. Class 3 Physical Form This is required of all adults 40 years of age and older attending summer camp, and is to be completed annually by a doctor. While it is not required for adults on weekend campouts, it is strongly encouraged.
